To join an SWI Disaster Assistance Response Team on a short-term missions trip, you'll first need to complete a Short-Term Mission Application and obtain recommendations from a pastor and a leader.  This can be done in one of two ways.
        1.  You can either print out the downloadable forms below and mail your application to our office with a 
            $100 check 
        - OR - 
        2.  you can fill out our online form and then either provide credit card information for the processing 
            fee or designate that you'll be sending a check.  You can also go to our donation page at a later 
            date and pay the processing fee through our secure website.
           
-A passport-sized photo is required with your Short-Term Application as well, so if you use our online version you will need to send a passport photo into our Home Office.
 
Follow-up calls from a representative of SWI will be made to pastors and leaders who make online recommendations.
